Print out the instructions, application, and state election board addresses (you can use your correct election board address if you live outside of Frederick County).

Fill out the Maryland Application for a Mail-In Ballot with pen. Read the instructions on the left side of the Mail-In Ballot application carefully. Be sure to sign the form.

You must mail or hand deliver your voted ballot. You cannot submit your voted ballot online, by email or fax. Mail or hand deliver the application to the Frederick County Board of Elections at its NEW location: 8490 Progress Drive, Frederick, MD 21701.
