Forms & Letter

      • Letter of Invitation to Candidate Forum  (In development)
      • PDF icon Debate Guidelines 2023 
        • This PDF file of document (any of the local leagues can use this document) contains the guidelines under which the forum will be conducted.
      • File Agreement to Participate - Template
        • This file links to a template to be used by the LWVDE Candidate Forum Committee or any of the local leagues who wish to use the template forms and letters.
        • The form requests the candidate, running for office, to participate in the LWVDE Candidate Forum 2023.
        • Committee members will insert data to replace the words in red found throughout the form.
        • Save As the document, using the name of the candidate as the file name.The file is emailed to the candidate.
          • Sample of Agreement to Participat form completed by LWVDE Candidate Forum Committee and sent to each candidate
          • File Sample of the Agreement to Participate completed by the candidate and returned to LWVDE. 
      • Video Release Form (file is being edited)
        • This file links to the template to be used by the LWVDE Candidate Forum Committee (or any of the local leagues who wish to use the template forms and letters.)
        • The form requests the candidate running for office to agree with the LWVDE statement.
Directions for using templates: Agreement to Participate and the Video Release Form
  1. Open the template.
  2. Use SAVE AS to save the file with the appropriate candidate's name.
  3. Replace all text that is printed in red with the correct information about the approaching Candidate Forum.
  4. If any text is still red, after you input the data, change the font color to black.
  5. SAVE the file.
  6. The candidates will be requested to fill in the information that is indicated by the grey font and return it to LWVDE at lwvde [at] comcast.net.
  7. There are 2 forms for each candidate to complete: the Agreement to Participate and the Video Release Form.
  8. Repeat these steps until each candidate has 2 forms to complete and return to LWVDE.
  9. Start at #1 above, until the templates for the Agreement to Participate and the Video Release form, are saved for each individual candidate.
  10. Attach 3 files saved for each candidate to an email with the Subject: Invitation to the Forum.
  11. Enclosures: Agreement to Participate, the Video Release Form, and the Guidelines for the debate.

Please DO NOT fill in the grey part of any form. The grey font indicates information required for the candidate to supply to the LWVDE.
